New Patients
Before meeting with a therapist you will need to fill out several documents. Expedite your initial visit to our office by completing the paperwork ahead of time. If you would rather fill out your paperwork in person, please arrive 15 minutes before your scheduled appointment.

PRINT & FILL OUT ALL FORMS, then bring them to your appointment:
- Patient Registration
- Adult Health History
- Pediatric Health History (Infants to age 16)
- Consent For Treatment
- HIPAA Notice

Insurance
Primary Therapy Source gladly accepts all insurance. Our clinic is a preferred provider for Medicare, Medicaid, Blue Cross and Regence Blue Shield.
Many patients come to our clinic because of their doctor’s recommendation. However, you do not need a physician’s referral in order to make an appointment for therapy, unless you are a client with Medicare or Medicaid.
Authorization for services is determined by each insurance carrier or Workman’s Compensation. Prior to your initial visit, contact your insurance company to learn about your eligibility and benefits for therapy services, along with any pre-authorization requirements.
We will bill your insurance company for you and direct you to insurance information sources. Feel free to contact our office for questions or assistance.
